Questions about COVID-19 vaccine cards? We have provided answers to two common questions and shared links to additional resources below. 

What should I do with my COVID-19 vaccine card?

  • Make a copy of the card and keep it in your wallet.
  • Take a picture of your card on your phone. 
  • Put the original card in a safe place at home.
  • Avoid laminating your card.

What should I do if I laminated or lost my card?

You can obtain a replacement card from the location where you received your COVID-19 vaccine or from your state health department. If you were vaccinated at a pharmacy such as CVS or Walgreens, you may be able to access proof of your vaccination on the pharmacy's website or app. If you were vaccinated at a health care organization, you may be able to access proof of your vaccination through their online portal. 

If you received your vaccination at an Advocate Aurora Health site, proof of vaccination can be found in your LiveWell account.
