Advocate Health and the Chicago Cubs have teamed up to offer 15 individuals with Down syndrome the opportunity to attend a meet and greet with a Chicago Cubs player! Participants will be able to get autographs and take photos with the player. The visit will take place on the morning of Thursday, September 19, on the campus of Advocate Lutheran General Hospital (exact location to be announced). Each participant will be invited to bring 1 adult guest. 

PLUS one lucky participant will get to throw out the 1st pitch at the evening Chicago Cubs game at Wrigley Field on Thursday, September 19! The participant and 3 guests will receive tickets to the game against the Washington Nationals.

If you would like the chance to attend the visit or throw out the 1st pitch, please fill out this form  by Wednesday, August 28. Participants will be selected randomly and contacted via email after the selection.

The Chicago Cubs have not announced which player will attend the event yet. We will share the name of the player as the event gets closer.

The Chicago Cubs have not announced the exact start time. It will be in the morning and will last about 1 hour.
