The Patient Advocate position is a unique opportunity to work with a multidisciplinary team to support adolescents and adults with Down syndrome and their families and caregivers. Job Title: Patient Advocate

Employment Type: Full time (40 hours), on site at the Adult Down Syndrome Center (1610 Luther Ln, Park Ridge, IL 60068)

Responsibilities:

• Connect patients and caregivers to services in their communities.

• Educate patients and caregivers about government programs.

• Develop and update resource lists.

• Coordinate patient groups, such as Music and Art Therapy.

• Support the overall mission of the Adult Down Syndrome Center to improve the health and lives of adolescents and adults with Down syndrome. 

• Other duties as assigned.

Position Requirements:

• High school diploma or equivalent with 3 to 5 years current related experience. 

• Strong interpersonal and telephone communication skills.

• Strong organizational skills.

• Experience with individuals with disabilities.
