Job Title: Patient Advocate
Employment Type: Full time (40 hours), on site at the Adult Down Syndrome Center (1610 Luther Ln, Park Ridge, IL 60068)

Responsibilities:

Connect patients and caregivers to services in their communities.

Educate patients and caregivers about government programs.

Develop and update resource lists.

Coordinate patient groups, such as Music and Art Therapy.

Support the overall mission of the Adult Down Syndrome Center to improve the health and lives of adolescents and adults with Down syndrome.

Other duties as assigned.
Position Requirements:

High school diploma or equivalent with 3 to 5 years current related experience.

Strong interpersonal and telephone communication skills.

Strong organizational skills.

Experience with individuals with disabilities.
