Resource Spotlight - My Health Passport

The phrase "health passport" is used to describe a collection of information related to one's health and medical care. There are different types of health passports including forms, documents, apps, websites, etc. They may contain information such as one's health history, family history, medications, communication preferences, and other needs. There are some versions that have been developed specifically for people with intellectual disabilities that provide information that is important for health care providers to know when caring for them.

My Health Passport opens in new window from the University of South Florida and the Florica Center for Inclusive Communities is one example of a health passport. Individuals with Down syndrome and their families can fill out the form and give it health care providers who may be unfamiliar with the individual with Down syndrome (e.g., a doctor at a hospital or urgent care clinic). My Health Passport opens in new window from Queensland Health in Australia is another example. It can be filled out online and then printed. 

During the COVID-19 pandemic, these types of documents were particularly helpful when individuals with Down syndrome may not have been able to get care from their regular health care provider.
