Staying Connected to Your Health Care Provider During the COVID-19 Pandemic

In light of the COVID-19 pandemic, many healthcare organizations - including ours - are encouraging patients to avoid coming to clinics and hospitals (when possible and if appropriate). Video appointments are being offered as an alternative. These visits are often accessed through health care organizations' online patient portals.

Online patient portals not only allow you to participate in video appointments but also access appointment summaries and lab results, send messages to your provider, and get health and wellness notifications. For patients of the Adult Down Syndrome Center, 

  • We encourage you to sign up for LiveWell, our patient portal. It can be accessed at this website or on phones and tablets using the LiveWell app. Information about proxy access for families and caregivers can be found at this link
  • If you are interested in scheduling a video appointment, please call our office at 847-318-2303. Additional information will be provided when you call to schedule an appointment. 
 

If you are not a patient of the Adult Down Syndrome Center, we recommend reaching out to your health care provider about patient portals and video appointments.
